and your call will be transferred to our scheduler by the operator or you may enter

3. What information will I need to provide to the scheduler?

Relevant demographics will be asked of you by the scheduler. Please provide the
following information to the scheduler:

Complete name and correct spelling.
Your mailing address.
Your telephone number for home and work if applicable.
Your marital status.
Your date of Birth.
Your insurance information (your primary and secondary insurance if
applicable).
Any prior testing, such as MRI or x-ray.
The name of referring physician, if applicable.
